Request Appointment

Please call the office at (508) 852-3700 to reschedule an appointment or request a cancellation.
Per CMPT policy, all cancellations made less than 24 hours from the scheduled appointment time will be charged a $35.00 late cancellation fee. All no-call/no-shows will be charged a $75.00 fee.
